Transition statements contain important information about each child's learning and development in kindergarten and include suggestions that will support each child's continued learning when they start school.
A transition statement summarises a child's learning in the five learning and development areas of the
Queensland kindergarten learning guideline or other equivalent and accredited learning guideline.
It is an expectation that approved and funded kindergarten providers complete a written
transition statement ​for each child enrolled in their program in November of the kindergarten year and provide this to families.
Parents, carers and children contribute to the transition statement describing their child's strengths, challenges, motivations and achievements.
Early childhood services, families and schools can use the information in the transition statement to discuss and plan for children's successful
transition to school.
Transition statement parent/carer consent
Parents/carers must complete a consent to create form (DOCX, 108KB) before the kindergarten teacher can commence a transition statement using the online application. Where a parent/carer does not consent to the transition statement being created in the online application, an alternative
kindergarten transition statement template (DOTX, 72KB) is also available.
Once the transition statement is drafted, parents/carers are provided a copy to review. Parents/carers are invited to make suggestions to support the continuity of learning for their child and request any incorrect personal information be amended. A parent/carer may give permission for a completed and approved transition statement to be shared with the school their child will attend the following year by signing a transition statement consent form (DOCX, 113KB). The purpose of the form is to make it easier for a child's transition statement to be passed to the school.
Parents and carers are given the consent form by the kindergarten teacher when they receive their transition statement. Transition statements belong to families who choose how they will share the statement.
The kindergarten teacher will inform them that by signing the form they can choose to:
- consent to the kindergarten sharing their child's transition statement with the nominated school on their behalf
- consent to the kindergarten and Prep teacher or other relevant school staff contacting one another to discuss information on the transition statement
- consent to the transition statement being shared with the outside school hours care service.
The consent form should only be completed
after families have read and support the transition statement. This ensures the family understands exactly what they are consenting to when they sign the form.
Finally, the kindergarten teacher can give permission to be contacted by the school by providing their details and signing the consent form.
