Approved providers must notify the Regulatory Authority of the removal or appointment of new committee members within:
7 days of any change that affects the approved provider's fitness and propriety
14 days of any appointment or removal of a person with management or control of an education and care service operated by the approved provider.
The approved provider must submit a notification of change to information about Approved Provider (PA08 form) online via the
National Quality Agenda IT System (NQA ITS). Please note this form can only be lodged online via the NQA ITS.
Approved providers are encouraged to submit 1 form when advising of the removal of or appointment of 1 or more persons with management or control.
Prior to submitting the required notification of change to information about Approved Provider (PA08 form) online, each person with management or control (PMC) is required to register with
Provider Digital Access (PRODA), which is used by the Regulatory Authority and the Australian Government to verify and authenticate their identity.

If the new person with management or control is a new member of the governing body or committee, the approved provider must also provide evidence to support the addition of a new person with management or control. Evidence of current officeholders could include, for example, in the case of:
- a company, a company report from the Australian Securities and Investments Commission which shows the current directors and secretary
- an eligible association, a copy of annual general meeting minutes that reflects the current office bearers.
The Regulatory Authority does not require a notification for existing committee members continuing on from the previous year. Their information is valid until they are removed, or the approved provider's fitness and propriety changes (e.g. a committee member's blue card is cancelled).
