The Positive transitions: early years toolkit is a practical, evidence-informed set of resources designed to strengthen educational transition practices across birth to Year 2. The toolkit provides flexible and adaptable resources that can be tailored to suit local contexts and priorities.
Based on
research developed by the University of Melbourne, the toolkit provides flexible and adaptable resources that can be tailored to suit local contexts and priorities. We recognise that positive transitions are a shared responsibility. The toolkit is designed to support all transition partners in creating responsive, collaborative, and developmentally appropriate transition experiences for every child. This includes resources to support families, community services, early childhood education and care services, and schools. Transition partners are encouraged to explore this valuable suite of resources.

Transition partnership placemat
The
transition partnership placemat is a conversation starter for transition partners to connect, discuss positive transitions for children birth to 8 years, and collaborate on shared roles and resources.
Transition planners
Transition planners are practical guides designed to help families, community services, early childhood education and care (ECEC) services, and schools to design and enact supportive, coordinated transition practices for children.
These tailored planners support transition partners to:
- navigate of transition pathways across birth to 8 years
- link to practical strategies and information
- collaboratively plan and build a shared language for early years transitions
- implement inquiry planning for ECEC and school settings.
Transition cards
A suite of practical transition strategies and practice examples to support transition planning. This resource can be used by ECEC services, state schools and community services.
Individual transitions guide
A statewide tool for transition partners to support children prior to school (birth to 5 years), who may require an individualised approach to transition successfully in their early years. It supports the team around the child to collaboratively plan a quality transition that is integrated over time and ensures the right support is in place at the right time.
This resource can be used by ECEC services, state schools and community services; and is intended to be used with families.
